Dear Mr, Steve Owen,

Last month we already sent you a warning letter asking you to improve your attendant behavior. Despite warning, there seems to be no improvement on your part.

Now on behalf of the company, I do not have other choice but to inform you that your services are no longer required after this month. If you wish to leave the company immediately, you always can go and your wage will be made up to the end of the month.

This is a forma dismiss an employee letter so it might be touch for you. Be short and do not elaborate more since you already sent him the final warning letter but nothing has changed from his part. However if there is warning letter previously, you need to go further in to the reason. State clearly the reason then tell him what he will get for what he has done for the company (wage).
